I have read and agreed to the terms and conditions of referral
I agree that primary care of the named referred patient remains my responsibility once discharged from Motion@Minster
Terms and conditions: Animals will only be seen on receipt of a written referral from the primary care veterinary surgeon. The referral must include full patient history, including all diagnostic imaging / recent blood results. The primary care veterinary surgeon remains responsible for all aspects of the animal’s care for any and all conditions outwith the reason for the referral. Responsibility for the animal (e.g. during a procedure for joint injections) is held by Motion@Minster solely for the period of being an inpatient, and reverts back to the primary care veterinary surgeon on discharge. Diagnostic tests and further investigations recommended in the initial consultation report are the responsibility of the primary care veterinary surgeon and results should be shared with Motion@Minster promptly to best enable patient care. An exception may be where diagnostics (e.g. plain radiography) are recommended concurrently with a therapeutic procedure (e.g. joint injection) to minimise anaesthetic procedures. In this event results will be shared with the primary care vets as soon as practicable. Payment will be expected at the time of treatment.
